The midlife "critical window" for your brain
A landmark 2025 study published in the Proceedings of the National Academy of Sciences (PNAS) analyzed brain scans from more than 19,300 people and found that brain aging doesn't happen in a straight line. It follows a predictable curve with clear turning points:
- Around age 44: Brain networks begin to destabilize, the first measurable stage of brain aging.
- Around age 67: Destabilization reaches its fastest pace.
- The driver: The researchers linked these changes most strongly to rising insulin resistance and poorer blood sugar control, not to blood pressure or inflammation. As neurons become insulin resistant, they struggle to take in glucose, their main fuel, and begin running short on energy.
- Ketones bypass the problem: Neurons take up ketones through a different doorway, the MCT2 transporter, which doesn't depend on insulin. Brain areas rich in this ketone transporter were more resilient to aging.
- Ketones restabilized the brain: In the study's intervention arm, 101 adults ages 20 to 79 received a single drink of D-BHB ketones or a calorie-matched glucose drink. Ketones produced robust, significant restabilization of brain networks, and the benefit was strongest in adults ages 40 to 59. The same number of calories from glucose had no significant effect.

- Ketones without the diet: The researchers note that exogenous ketones raise blood ketone levels quickly and in a dose-dependent way, even when eaten alongside carbohydrates, with no currently known safety concerns.
- Ketone minerals and mood: In a 6-week, double-blind study from The Ohio State University published in Frontiers in Neuroscience, adults following a ketogenic weight-loss diet took BHB bound to minerals, the same type of ketone used in Brain Ketone, or a placebo twice daily. At week 2, the placebo group had higher depression scores and lower scores on a math processing test than the ketone group. The researchers concluded that the ketone supplement may help ease the negative mood that can come with the early "keto-adaptation" phase.
Ketones and migraines: what the research shows

The researchers concluded that ketones may be most helpful for people whose migraines are linked to metabolic and energy imbalances.
The science is still young. Most of the evidence so far comes from ketogenic diets, the studies are small, and larger trials are needed.
